SAFETY NET APPEAL FORM DUE TOMORROW (JUNE 11)!

GOOD EVENING,
As a reminder,  the DSRIP Safety Net Appeal form is due by 5 p.m. tomorrow, June 11, 2014. 

The state Health Department asks that appelants complete their appeal form in Microsoft Excel and attach it in an unsecured e-mail, as follows:

• with the subject line of “(Appeal Facility Name--fill in name of appealing institution) – DSRIP Safety Net Appeal” and send it 
• to BVAPR@health.state.ny.us

Late appeals will not be accepted.  Questions and submitted appeals should be emailed directly to BVAPR@health.state.ny.us.
