DSRIP Project Design Application IS DUE by 5 p.m. Today

Good afternoon,

As a reminder to all providers, the Project Design Grant Application is due today, June 26, 2014 at 5pm to BVAPR@health.state.ny.us. 

The Health Department sent out reminders today that the forms must be formatted in the same way they were created in order to allow data collection from the forms, and asked that potential grantees LEAVE TABS ALONE and NOT add lines that could cause data to shift down rows.  So, DO NOT do any of the following:

§  Add tabs to the forms

§  Move around the tabs

§  Inserting lines above the column headings – This causes to shift the rows down of where the data was originally supposed to be.

They also remind those submitting the Design Grant Application, these three documents should be sent separately:

(1)     Project Design Grant Application

(2)     Updated Section 2 form

(3)     Section 10 – Project Advisory Committee (PAC)
