Lead PPS Providers Must Submit New Section 2 Form for Intent Letter

If you submitted a Letter of Intent (LOI) in “Excel” and your PPS was categorized as an Emerging Performing Provider System (PPS), the Department of Health has sent the lead contact a newly formatted Letter of Intent containing names and information about their partner organizations.

Changes were made and posted June 18 to allow ease of use for copying and pasting data onto the new updated Section 2 form. Further information can be found in the instructions.  The NYS DOH expressed that it knows the redux will be time-consuming but that it is essential to the Project Design Grant Application process.

·         An “Updated” Section 2 form is posted on the state's DSRIP website along with instructions. The new form consists of the following tabs:

o   Section 2.1 – This tab is for the following provider types ONLY – Assisted Living facility, Certified Home Health Agency, Critical Access Hospital, Diagnostic & Treatment Center, Federally Qualified Healthcare Center, Long Term Home Health Care Provider, Nursing Home, OASAS Article 32 Provider, OMH Article 31 Provider, OPWDD Article 16 Provider, Public Hospital, Skilled Nursing Facility, Sole Community Hospital, and Voluntary Hospitals (non CAH, Non SCP).

o   Section 2.2 – For providers not listed in Section 2.1, all other provider types should be listed here (Except for Physicians and Pharmacies which should be listed in 2.3 and 2.4).

o   Section 2.3 – All Physicians should be listed here (Includes: Certified Nurse Midwifes, Dentists, Nurse Practitioners, and Physician Assistant**

**A small change has been made to this section since the Webinar on 6/18 - “Physician Assistant practicing in FQHC or RHC led by a PA” has been change to “Physician Assistant”).

o   Section 2.4 – All Pharmacies should be listed here
