As you begin coordinating projects through the Delivery System Reform Incentive Program, the state Health Department is posting region-specific maps and charts of Medicaid Avoidable Hospitalizations and Clinical Process of Quality Measures for DSRIP provider reference online, as follows:

• Western New York region information on Medicaid avoidable hospitalizations 
• Clinical Process of Quality Measures (page through document to view various measures)
